According to a study conducted in 2000 by the National Academy of Science’s Institute of Medicine, medical errors have been one of the leading causes of death in the United States.

DrFist, a company that helps to maintain an electronic prescription management system for health care professionals, claims that nearly 7,000 of these deaths are due to prescription errors.

Medical Reconciliation

According to researchers, hospitals need to have access to an updated list of each patient’s medication history in order to properly care for them.

This process is known as medical reconciliation.

Medical reconciliation is when health care professionals compare the medications that patients have been prescribed in the past to the medications the patient may receive when admitted into the hospital.

New System Studied

Studies have found that most prescription errors are due to illegible handwriting, wrong dosages being given and patients receiving drugs that conflict with other medications they are taking.

Fauquier Health System is one of the first health systems in the country to begin studies regarding a system that incorporates a patient’s current prescription from other sources.

Aware of the growing problem with prescription error across the country, Fauquier Hospital is attempting to develop an electronic prescription system that is enabled by outside resources to exchange prescription information.

With the new system, no matter what unit of the hospital the patient is admitted to, doctors can search their databases to see the patient’s prescription history.

Fauquier Hospital sees nearly 123,000 patients annually and this new “paperless” prescription system is aimed at ensuring patient safety to prevent errors from occurring in the future.

Researchers report that before the 90- day trial study with the new technology is done, nearly 1,500 patients will already have benefited from it.
